Clearance Instructions
SSOM clearance instructions for excused absence from class/clinicals:
Covid testing should be completed if you exhibit even one of these COVID symptoms: Fever, sore throat, cough, runny nose, nasal congestion, change to sense of taste or smell, shortness of breath, fatigue, or body aches.
If you feel sick and will be missing class/clinicals because of upper respiratory symptoms:
Step 1 Immediately do a home COVID test.
Step 2: Write your name, date of birth, date and time of your test on the cassette.
Step 3: Take a picture of the test cassette results and email it to wellnesscenter@luc.edu.
Step 4: After you have completed your test and sent it in, call Dial-a-Nurse 773 508-8883 to obtain an excuse note. If the nurse is on another call, please leave your name and phone number and a nurse will return your call.
If your first test is negative, you may be asked to repeat the home COVID test in 48 hours. If you are advised to repeat your covid test, please follow the above instructions. After you have completed and sent in your second COVID test, call Dial-a-Nurse at 773 508-8883 to schedule a phone clearance appointment to return to class/clinicals. If you do not answer the phone when called, you will have to reschedule your appointment.
Home tests are available for purchase at the front desk of the Wellness Center for $4.00.
If you feel sick with something other than an upper respiratory infection, please call Dial-a-Nurse at 773 508-8883. If the nurse is on another call, please leave your name and phone number and a nurse will return your call.
- If you need a clearance to return to class/clinicals when the Wellness Center is closed, you can go to another provider to request a clearance note (primary care, urgent care, minute clinic, etc). That appointment would be through insurance and may require a copay or an office visit fee. This provider cannot be a relative.
For preexisting/chronic conditions:
Students with preexisting or chronic conditions should provide relevant documentation from their primary care provider to the Office of Student Affairs in order to obtain medically necessary clearance from academic responsibilities. Once provided, contact OSA for future clearance requests that are pertaining to preexisting or chronic condition.
